Coaches’ (CORI) KidSafe forms
All coaches and assistants must complete a CORI background check by filing a CORI Background Information form with the Massachusetts Youth Soccer Association (MYSA). For security purposes, you can only fill out this form on line at MYSA. Once a CORI form is filed, it remains in effect for 5 years. The Registrar will notify all coaches and assistant coaches if and when they need to complete the CORI form.
MYSA mandates that background checks be done for all volunteers in affiliated soccer programs, including the Southborough Youth Soccer Association (SYSA). The background check will review a person's entire criminal record history, with the exception of those cases where the records are sealed. Visit the MYSA CORI Policies and Procedures for more information on the MYSA KidSafe program.
KidSafe Registration also provides you with the blanket liability policy required if you are a coach or administrator. Remember that no one will be allowed to coach or assist without a KidSafe registration.
